Tradition

Frankincense resin has been gathered and traded for 5,000 years. Across Egypt, Mesopotamia, Arabia, and India, it served as sacred incense, embalming material, and medicine for joints, digestion, and the respiratory tract. In Ayurveda it is known as Shallaki and prescribed for inflammatory joint conditions in texts dating to 600 BCE.

Modern evidence

The boswellic acids — particularly AKBA — are selective 5-lipoxygenase (5-LOX) inhibitors. This distinguishes Boswellia from NSAIDs, which primarily target COX enzymes. A 2018 meta-analysis of six RCTs found significant pain reduction and functional improvement in knee osteoarthritis, with effects visible at 30 days.

How to choose a product

Quality matters enormously. AKBA is the most active compound and is poorly absorbed without enhanced formulations. Look for products labelled '5-Loxin' or 'Aflapin' (both contain phospholipid-enhanced AKBA). Take with a fat-containing meal to significantly improve absorption.
